Use of Personal Data in Connection with my Teaching Services

What I need

Prior to/At your first music lesson, you will be asked to complete a data consent form which will ask for the following information below.  Unless otherwise agreed with you, I will only collect basic personal data about all my students plus any parents/guardians if the pupil is under the age of 16. The information I need can be found below.:

1) Names of my students (and parents/guardians if the pupil is under the age of 16):

2) Home address

3) Email addresses (parents/guardians email addresses if the pupil is under the age of 16):

4) Telephone numbers (parents/guardians telephone numbers if the pupil is under the age of 16):

Other personal information below is not required by me, but is useful to have, for reasons that are next to each point.

5) Date of birth/age of student: (This is needed when I enter pupils for exams. Also it can help me to find age relevant materials, for example a 10 year old may prefer to play current music rather than music from the 1950’s)

6) Medical needs/conditions: (Only provide me this information if you feel I need to be aware of anything)
